چکیده

Background: As supply chain risks affect its output, and any disruption or risk in the supply chain has a direct impact on company operations and products or services delivery to the market, it seems necessary to identify and manage the risks in the supply chain in order to make proper balance throughout the supply chain, provide the necessary resources and finally deliver products to customers, and it can be considered as one of the requirements for the successful management of this area. Purpose: This study aimed at providing a model and identifying the most important risk factors of supply chain in the dairy industry, and then evaluating the proposed model in an active company in the dairy industry. Methodology: In the proposed methodology, the risk factors of supply chain is divided into seven main criteria and thirty seven sub- criteria, and then the confirmatory factor analysis is used to verify the measurement model. Then, the importance and weight of each risk factor is measured by using fuzzy hierarchical analysis process, and the level of each risk factor is calculated using a five-point Likert spectrum. Finally, supply chain risks are evaluated by using final weighted score and basic score for each factor. Results: Seven major factors in the dairy supply chain risks include: "political risk, market risk, technology and IT risk, disruption risk, supply risk, the risk of the production process and communication and cooperation risk". These factors have thirty seven sub-criteria which are obtained using confirmatory factor analysis. Results show that the supply risk has the highest weight among seven main risks. Conclusion: Finally, according to the findings of the study and Weight-Risk chart, risk factors are divided into three categories of unacceptable, tolerated and accepted risk.
